School Forms

Dear Parents,

Please read all of the below information carefully, complete the necessary forms and return them to Jillian Mayonove at jmayonove@parksideschool.org. It is extremely important that we have the latest contact information for your family including who to contact in an emergency or illness if you can’t be reached. Please list someone as an emergency contact who will be able to come to the school to pick up your child within one hour of being called.

Department of Health regulations require that a child not start the new school year if all health forms have not been returned and if your child’s immunizations are not up to date. We must also have documentation for medications to be given at school.

Thank you for your cooperation in returning all of this information promptly.

You may sign them electronically and submit them online, or print them out and return them. Please note that to complete the forms electronically, you will need to save the files to your desktop and open them in Adobe Acrobat. Once completed, save the forms and email them to jmayonove@parksideschool.org.

REQUIRED MEDICAL FORMS

Please review this Letter to Parents from Parkside’s school nurse, Morlon Mitchell.

If at any point your child will need medication at school—including over the counter medication—a medication administration form will need to be filled out by their healthcare provider.

• Required Medication Administration Forms only students who will need medication at school either daily or as needed:

  • General Medication Administration Form

  • Asthma Medication Administration Form

  • Allergies/Anaphylaxis Medication Administration Form

  • Seizure Medication Administration Form

The Medication Administration Forms do not need to be returned unless your child will need medication at school (either daily or on an as needed basis). If your child takes medication at home and it is not indicated on their physical exam form, please contact Nurse Mitchell.

Once completed, forms should be returned to Jillian Mayonove.
